In the acute stage of bartonellosis, specific IgM antibodies can be detected at the earliest 20 days after infection. If clinical suspicion persists despite negative results, screening via Bartonella henselae EliSpot and determination of CD3+/CD57+/CD56+/CD45+ cells is recommended.

Bartonellae are gram-negative, facultatively intracellular bacteria capable of causing long-term infections. Target cells are endothelial cells and erythrocytes. The most common subspecies in Central Europe are Bartonella henselae ("cat scratch disease" - transmitted via cat scratches and ticks) and Bartonella quintana ("trench fever"/"five-day fever" - transmitted via body lice). Incubation period: 3-38 days.

Typical symptoms: skin papules, skin redness, regional lymph node swelling. Atypical symptoms: fatigue, headaches, sweating, muscle cramps, muscle twitching/tremors, morning fever (up to six-week phases), chills, skin pain, swollen lymph nodes, joint pain (arthralgia), muscle pain, sleep disorders, depression, inner restlessness, severe mood swings, difficulty concentrating, dizziness, anxiety, outbursts of anger, antisocial behavior, restlessness, gastritis, intestinal disorders, painful soles of the feet (especially in the morning), painful subcutaneous nodules on the limbs.
