If clinical suspicion of borreliosis persists despite a negative antibody test, the antibodies should be retested after 2 weeks if necessary, as they sometimes develop with a delay.

Important:
- Antibodies indicate an infection but not necessarily an active disease.
- Antibodies can disappear again within a few months.
- In some cases, antibodies persist for years despite therapy.

Although the Borrelia IgG/IgM antibody ELISA is part of the routine workup, it is not sufficiently conclusive on its own, as antibody formation may be absent under certain circumstances. Therefore, following a negative test but with persistent symptoms, it is recommended to perform screening using the Borrelia EliSpot, Tickplex Basic, and CD3+/CD57+/CD56+/CD45+ cells.

Borrelia-specific antibodies can be detected in the blood at the earliest 20 days after a tick bite. In the early stage of borreliosis, these are usually only specific IgM antibodies. If the borreliosis is already advanced, specific IgG antibodies are also present.
